Case 004 — Alarm Suppression and Safety Envelope Drift

Context
A complex industrial or operational system is supervised through an alarm management framework. Operators rely on alarms and alerts to identify deviations from expected system behaviour. To reduce alarm fatigue, alarm thresholds and filtering mechanisms are periodically adjusted so that only the most significant conditions trigger alerts.
Situation
Over time, operational conditions gradually shift due to normal variations in system usage, environmental factors, or configuration changes. The alarm management system continues functioning correctly according to its configured thresholds. No alarms are triggered. Operators observe stable system status and no abnormal warnings. However, the conditions under which the alarm thresholds were originally defined no longer fully represent the actual safety envelope of the system. The system therefore operates closer to operational limits without generating alarms.
Boundary
The monitoring and alarm system has not failed. All configured logic operates correctly. However, the assumptions used to define the alarm thresholds and safety margins are no longer fully valid. This condition represents an Applicability Boundary.
Key Insight
Applicability boundaries may arise when monitoring frameworks remain technically correct while the operational safety envelope they represent has gradually drifted. In such situations, the absence of alarms does not necessarily indicate the continued validity of the underlying operational model.
